Assisted reproductive techniques for infertile couple

The couple , who is unable to conceive naturally after one year of regular intercourse, is labelled as infertile. This couple seeks the help of infertility specialist.  Investigations are done in the couple to find out the reason of infertility . These investigations are targeted towards both male and female partners ( mentioned in my previous blog dated 9th February 2024 )After finding out the reason , reproductive specialist decides for the best treatment option for the couple, out of the available treatment modalities .

Various treatment modalities available are -

1.Controlled ovarian hyper stimulation: The female partner is given medicines , starting from the 2nd/3rd day of menses to stimulate  the ovaries for the production of good quality eggs capable of giving pregnancy. The follicular growth is monitored with transvaginal sonography during controlled hyper stimulation and when egg/eggs are around the size of 18-20 mm , trigger with HCG or Decapeptyl is given. Timed intercourse results in conception.


2.IUI: Intrauterine insemination is the direct placement of processed highly motile concentrated sperms into the uterine cavity. Sperms before placement are washed free of seminal plasma


3.IVF: In In vitro fertilization , the eggs and sperms are fertilised in the laboratory by placing eggs and sperms in special media in an incubator, under strict controlled environment.


4.ICSI:Intra cytoplasmic sperm injection is done when sperms fail to fertilize the eggs naturally . The fertilization of an egg is achieved by injecting a single sperm into the cytoplasm of an egg with the help of a special machine called micromanipulator.


5.ET: Embryo transfer is the process of transferring the embryos, which have been developed after IVF/ICSI in the lab, into the uterine cavity for implantation.


6.Ovum Donation: When the woman loses the capacity to produce her own eggs, then eggs are borrowed from a healthy fertile young married female, who already has child/ children. Government of India allows altruistic donors or the eggs are to be taken from the registered oocyte banks.

7.Surrogacy: It's allowed in the females who have badly damaged or absent uterus. Only altruistic surrogacy is allowed as per the laws.

As per the Indian laws , all assisted reproductive techniques are allowed in the couples , where the age of the female partner is less than 50 years and the age of the male partner is less than 60 years.
